What Is Hospice Care?

Hospice care is specialized end-of-life care designed for individuals with a terminal illness who are expected to live six months or less if the illness runs its normal course. Once patients stop pursuing curative treatment, hospice focuses on providing comfort and support rather than seeking a cure.

Polaris Hospice takes a patient-centered approach, helping manage symptoms and provide treatment for pain, nausea, breathing difficulties, and other challenges. Our care team includes physicians, nurses, social workers, hospice aides, volunteers, and chaplains,all working together to address physical, emotional, and spiritual needs.

How Long Is Hospice Care?

The length of hospice care varies. Some patients receive hospice for weeks, others for several months. In some cases, individuals live beyond the initial prognosis and continue receiving care if they still meet the eligibility criteria. Patients may receive routine visits from our hospice staff in their home, a nursing home, or an assisted living facility, depending on where they reside.

How Do You Qualify for Hospice Care?

To qualify for hospice care, a patient must have:

  • A terminal diagnosis with a life expectancy of six months or less
  • A decision to stop curative treatment and focus on comfort and quality of life
  • Progressive symptoms such as weight loss, frequent infections, or repeated hospitalizations

Hospice eligibility is usually determined by a physician, but family members can initiate conversations with healthcare providers if they believe a loved one may benefit from hospice. Common conditions that qualify include cancer, dementia, congestive heart failure, COPD, and other serious illnesses.
